To assist us in providing you with a high quality service we request you complete the questionnaire accurately and return to us in advance of your initial appointment. The information provided assists us in making recommendations about the treatment of your hip condition. Should surgery be planned, the information will be forwarded to your anaesthetist to assist in the safe planning and conduct of your anaesthetic.
